Permission To Skate Forms
Permission to Skate forms (PSF) for AAA and JR teams will be available Sunday April 14th at the LMHA Banquet.
Players are required to pre-register with their Home Centre as per the OMHA Manual of Operations.
Please come prepared to register your player.
