Late Registration
Local League Late Registration is
still being accepted for most divisions. To complete registration for your
child, you will need to complete the registration form and provide full payment
(including fundraising fee) or meet one of the payment options with a postdated
cheque. If your child has not played for LMHA in the past, a copy of birth
certificate must be provided. All registrations must be in person.
Space is
limited and is on a first come basis.

At least one parent or guardian per player must have this course by August 31, for player to be approved to a roster for upcoming season. The program can be accessed through the OMHA website (www.omha.net) with a cost of $12 (payable through the site). It is a one hour online curriculum with both audio and visual features that allows flexibility and does not have to be completed in one session.
Fundraising Fee
2014/2015 fundraising fee is $125.00 per family. Upon registration, a book of 25 x $5 raffle tickets must be purchased. This year’s raffle includes 3 TV packages, provided by Earl Kennedy Electric, 10 $500 cash prizes and 1 $1000 cash prize. Payment for tickets is required at the time of registration. Tickets will be distributed with registration.
Note: If you are registered and have not received your fundraising tickets, you will receive them in September from your Team Manager.
Mandatory Volunteerism
LMHA is run by volunteers at every level. With your child’s registration, you have an obligation to volunteer for an LMHA event. Volunteer hours are distributed amongst each LMHA team. In September, teams will be provided with a schedule to fulfill at the LMHA Food booth operated during the Lindsay Ex fall fair.

Child Fitness Tax Credit
Completed registration forms with LMHA board member or appointee initial acknowledging receipt of payment may be used to claim Child Fitness Tax Credits for the tax year. The portion related to fundraising fees ($125) is not eligible for tax credits.
Registration Division & Level
Player’s wishing to register for an above division or representative level must pay registrations corresponding with the division or level in which they are registering for. Should the player be deemed ineligible, by LMHA board or appointee, for the registered division or level, reimbursement of the difference will be issued in September.
All players attending tryouts must be registered with payment at the appropriate level.
E.I. 3rd year Mite aged player wishing to participate in the Tyke Assessment must register “Tyke” or Atom aged player wishing to attend tryouts must register under the BB rate.
