End of Season Notice to Parent/ Players/Coaches
To All LMHA Members
As the end of the 2014-2015 season is quickly coming upon us you are reminded to complete you’re Coaching Evaluation Form for Parents and Players. The forms are available on-line at the LMHA Website under “LMHA Forms”. Parents
please use this form to provide feedback on your 2014/2015 season.
Place completed form (labelled Rep-AE or Local League) in the LMHA
mailbox in Pad 1 beside the LMHA Information Board at the Lindsay
Recreation Complex.
