TO SKATE OR NRP PASSPORTS 2015/2016

LMHA is excited to announce, new this year,
online request process for permission to skate forms and Non-Resident Player
Passports!
In keeping with, OMHA Manual of Operations,
players are required to pre-register with their Home Centre.
Players wishing to register with LMHA, for the sole purpose of trying out for another organization, must complete the online NRP & PTO Pre-Registration Form.

Pre-Registration Forms submitted in advance of April 12th, will be available to pick up during the LMHA Awards Banquet, at LCVI.
Forms submitted after will be processed and available for pickup Lindsay Sportsline, 32 Kent St W, Lindsay, ON K9V2Y1. M-F between 9:30-6 or Sat between 9:30-4.
