JR Permission to Skate Form Request

A JR PTS (Junior Permission to Skate) form is required for U18 players wishing to attend Junior Camps. Please fill out a separate request form for each player requesting these documents (if you have multiple players in the same residence). ***JR PTS Requests require at least 48hrs to process from submission date***
