Covid-19 Screening, News (Lindsay Minor Hockey)

As our ice times begin, it’s important that EVERYONE must complete the online Covid-19 Screening prior to each & every ice time before entering the arena!

Each age division has their own screening tool so Covid Reps can verify completion prior to every ice time. These screening tools are located in a couple of places on the LMHA website to make it easy for members to find:

• “Covid-19 Screening Tools” drop down menu: website main page menu bar (click age division to be redirected to the appropriate screening tool)

• Top of individual age division’s LMHA web page (click age division from the “Teams List” to view clickable “Covid-19 Screening” banner & QR code: both will redirect you to to the appropriate screening tool)

• From Mobile: click “Organization Menu” then “Covid-19 Screening Tools” - or - click “Team Finder” then age division (swipe to toggle between QR code or clickable banner)

• QR codes can be printed out by Covid Reps for use at the arena during check-ins as well

Coaches: please check your emails. A Covid-19 Screening Tool link & Covid Rep Tracking Link, as well as a QR code for your individual age divisions were sent to you last night!

COVID-19 SCREENING TOOLS:

• Last Name Letter grouping doesn't matter, as long as you fill out the correct age division

• Click to open the Google Form & complete the screening. Don't forget to click submit!

• If your screen is GREEN, proceed to check in with your Covid-Rep at the Pad 2 doors. Once they’ve confirmed your screening results, you may enter the arena 

• If your screen is RED, DO NOT enter the arena! Isolate, alert your Covid-Rep & public health

• Repeat screenings for each Participant & Spectator entering the arena. Coaches & all volunteers must complete a screening as well

 

How-To Scan a QR Code:

• Need a Wi-fi or Data internet connection

• Ensure your device settings have the QR reader capabilities enabled (this is automatic on most cellular phones)

• Open your device camera & center over the QR code to be scanned(as if you were taking a picture)

• A notification should pop up to redirect you to a website (in this case, the Covid-19 Screening Google Form for your age division)

***These Covid-19 screening tools are temporary for the pre-skates during the month of September only. Permanent season screening tools will be implemented in October and will be found in the exact same places as these ones, but tailored to individual teams***
